Position Description

OVERVIEW:

The RN at Cheyenne Village supports the health and well-being of individuals with intellectual and developmental disabilities through clinical oversight, health assessments and care coordination. This role requires collaboration with interdisciplinary teams, families and external healthcare providers to ensure high-quality, person-centered care in residential and community-based settings.

ESSENTIAL JOB FUNCTIONS:

  • The RN conducts health assessments and develops individualized care plans, is responsible for medications and maintains the medication Administration Record (MAR), monitors chronic conditions and responds to acute health concerns, provides health related training and guidance to staff, maintains accurate and timely documentation in the electronic health record(EHR), communicates effectively with families, guardians and healthcare professionals, participates in interdisciplinary team meetings and ensures compliance with state regulations, agency policy and nursing best practices.
  • The RN reviews provider notes to ensure appropriate follow-through and adjusts treatment plans as needed. RN is responsible for coordinating medical appointments, follow-up, and hospital discharges in collaboration with staff and the nursing records secretary, ensuring timely communication with physicians, specialists and pharmacies when care-plans or recommendations change. The RN also prepares laboratory requisitions and outpatient procedure documentation as required.
  • Participates in monthly medication audits to ensure accuracy and compliance, identifying and addressing errors before distribution. Communicates with Team Leaders and Direct Support Professional’s (DSP) as needed when issues arise.
  • Educate staff, clients, and families on health-related topics, including medication administration, nutrition, hygiene, and disease prevention. Provide training and oversight to direct support professionals (DSPs) on health protocols and emergency procedures.
  • Advocates for client’s health rights and access to services while ensuring compliance with agency policies, HIPPA, OSHA and nursing standards. Participates in audits, quality reviews and incident investigations as needed.
  • Maintains and distributes health service supplies.
  • Performs all other job duties as assigned.

LICENSES/CERTIFICATIONS:

Current and unrestricted Colorado RN license.

Must maintain a valid driver’s license and acceptable driving record per Cheyenne Village and insurance requirements.

Maintain certification in CPR and First Aid.

 QMAP Instructor Certification may be required.

WORKING CONDITIONS:

Combination of normal office environment, residential and community settings. Occasional travel to client homes, medical appointments and care facilities.

PHYSICAL DEMANDS:

Works in a standard on-site business office environment. Exerts force and/or lifts or carries objects up to 10 pounds occasionally (i.e. storage box with files).

STATUS: Exempt FT 30 hours per week.

PAY TYPE: Salaried, Monday-Thursday 7 1/2-hour days with Friday, Saturday and Sunday off.

We offer benefits 1st of the month following 60 days of service. (Medical, Dental, Vision, 401k Retirement, Life/Add, Voluntary Life and FSA).

We do conduct a background check prior to employment to include DMV, CBI and CAPS checks.
